About Maple Health Primary Care

Maple Health Primary Care aims to provide you with the care and attention you deserve! Our goal is to provide you with preventative care and care when you are ill. We will listen to your needs and work on a treatment plan together. We are excited to provide care for chronic medical conditions such as weight management, hypertension, diabetes, anxiety, depression and many others! We will strive to provide same day or next day appointments when you are not feeling well.

You will have access to your test results and will be able to communicate with your doctor’s office through our portal which is powered by Athena. We will streamline checking in for your appointment to minimize the amount of documents to complete before you can even see your doctor! You will be able to make appointments and view your bills through our portal!

About Dr Panarker

Dr Panarker is a Board Certified Family Physician and is a Fellow of the American Association of Family Physicians. She is also on staff at Ascension Saint Alexius Hospital and Northwest Community Hospital. She has provided care to several patients and their families for a decade in the Palatine and neighboring community.

About Dr Katz Pham

Dr. Katz Pham is a Board-Certified Family Physician working in the Northwest Suburbs since 2010. She is on staff at two local hospitals, Ascension Saint Alexius and Ascension Alexian Brothers. Prior to joining Maple Health Primary Care, she worked for Ascension Medical Group in Palatine, Illinois.
