About Maple Health Primary Care
Maple Health Primary Care aims to provide you with the care and attention you deserve! Our goal is to provide you with preventative care and care when you are ill. We will listen to your needs and work on a treatment plan together. We are excited to provide care for chronic medical conditions such as weight management, hypertension, diabetes, anxiety, depression and many others! We will strive to provide same day or next day appointments when you are not feeling well.
You will have access to your test results and will be able to communicate with your doctor’s office through our portal which is powered by Athena. We will streamline checking in for your appointment to minimize the amount of documents to complete before you can even see your doctor! You will be able to make appointments and view your bills through our portal!
About Dr Panarker
Dr Panarker is a Board Certified Family Physician and is a Fellow of the American Association of Family Physicians. She is also on staff at Ascension Saint Alexius Hospital. She has provided care to several patients and their families for a decade in the Palatine and neighboring community.
Dr Panarker was born and raised in Canada. She graduated with an Honors Bachelor of Science from University of Toronto Mississauga. She was awarded an Academic Distinction during her tenure at University of Toronto Mississauga. Dr Panarker pursued her Medical Degree from Saba University School of Medicine in the Netherland Antilles and received Academic Honors. Dr Panarker completed her Family Medicine Residency in 2012 from Oakland University William Beaumont School of Medicine (Troy, Michigan).
Outside of her busy medical practice, Dr Panarker enjoys reading a good book and exercising. Her life is full of surprises and fun activities with her husband, two daughters and son. In her free time, she makes every attempt to stay connected with her family and support network.
About Dr Katz Pham
Dr. Katz Pham is a Board-Certified Family Physician working in the Northwest Suburbs since 2010. She is on staff at two local hospitals, Ascension Saint Alexius and Ascension Alexian Brothers. Prior to joining Maple Health Primary Care, she worked for Ascension Medical Group in Palatine, Illinois.
Dr. Katz Pham earned her Bachelors of Science Degree from the University of Illinois Urbana-Champaign, graduating with the highest academic achievement honor of Bronze Tablet distinction. She then received her Medical Degree from Southern Illinois University School of Medicine in Springfield, Illinois. In 2010, Dr. Katz Pham completed her Family Medicine Residency at Lutheran General Hospital in Park Ridge, Illinois. She served as co-chief resident during her final year.
Outside of her busy medical practice, you can find Dr. Katz Pham spending time with her husband and three boys. They love playing and watching sports together but also enjoy some more relaxing television binging and family traveling adventures.
